1. Acknowledgment of Risk
I, the undersigned parent or legal guardian, hereby grant permission for my child, ___________________________ (the “Participant”), to participate in The Perfecct Me Children’s Sporting Event (the “Event”). I understand and acknowledge that participation in sporting activities involves inherent risks, including, but not limited to, physical injury, emotional distress, and other unforeseen incidents.
2. Assumption of Risk
By signing this waiver, I voluntarily assume all risks associated with my child’s participation in the Event. I understand that The Perfecct Me, its staff, volunteers, and affiliates have taken steps to ensure safety, but that accidents or injuries may still occur despite these precautions.
3. Release of Liability
In consideration of the opportunity for my child to participate in the Event, I agree to release, indemnify, and hold harmless The Perfecct Me, its officers, directors, employees, volunteers, sponsors, and agents from any and all liability for any injury, loss, or damage incurred by my child or myself, whether arising from the negligence of any of the above or otherwise, to the fullest extent permitted by law.
4. Medical Authorization
In the event of an emergency, I authorize The Perfecct Me and its designated representatives to secure necessary medical treatment for my child. I agree to be responsible for any costs incurred from such medical care.
5. Parental/Guardian Certification
I certify that I am the parent or legal guardian of the Participant and have the authority to sign this waiver on their behalf. I further certify that I have read, understand, and agree to the terms and conditions outlined above. I acknowledge that by signing below, I am giving up certain legal rights, including the right to sue The Perfecct Me and its representatives for any injuries or damages arising from my child’s participation in the Event.
